General
In order to provide good medical care, we record your contact and medical details. We do this in accordance with the Medical Treatment Agreement Act (WGBO) and the General Data Protection Regulation (AVG). This means that we only use your details for the provision of medical care and associated administration, and that we secure your details against infringement by third parties.
We will keep your data for as long as is necessary to provide care and as long as the law requires us to retain your data. After this period, we will delete your data.
We exchange data with other healthcare providers, such as GPs, pharmacies, hospitals, physiotherapists, etc. We do this via secure systems and only when necessary. We ask for your consent before we make your medication data available to hospitals and pharmacies via the national LSP.
Our employees and GPs only consult your data when it is necessary for the performance of their duties.
To monitor and improve the quality of our services, we record telephone conversations. These recordings are deleted after a maximum of five years.
For your and our safety, camera footage of the waiting area is recorded. This footage is not stored.
We have engaged ICT suppliers for the maintenance and management of our information provision. We have carefully selected these suppliers and made clear agreements with them regarding confidentiality and security. Should it happen, despite our measures, that third parties gain unauthorised access to your data, we will report this to the Autoriteit Persoonsgegevens (Dutch Data Protection Authority).
We are querying the Registered in Whose Name (ION) database
If you report to the out-of-hours GP service with an urgent care need, we will note which GP practice you are registered with and send your GP a message about the treatment received at the out-of-hours GP service. This will enable your own GP to follow up on your care. We want to inform the correct GP practice about your treatment. Therefore, we verify this in the Registered Name database, which is managed by the Registered Name Foundation. GP practices themselves record who is registered with them in this database. See www.inschrijvingopnaam.nl.The database contains no medical information about you. It only states that you are registered with your GP practice. If you have any questions about this, please contact Stichting ION: www.inschrijvingopnaam.nl/contactformulier.
Your rights
You are entitled to access your data. If it turns out that data about you is incorrect, you have the right to have us correct or delete it. It may not be possible to fully comply with a request in certain circumstances (for example, if your access leads to a breach of the privacy of others).
Should you wish to view your data, you can contact our Data Protection Officer, J. Janssen, about this.
Complaints
If you have any complaints or compliments regarding the way we process your data, please contact our Data Protection Officer. Should you not reach an agreement with this officer, you have the right to lodge a complaint with the Dutch Data Protection Authority.
